   Hello has anyone setup a form to take client reviews for a service company for example. Simple example is rate our service by answering thse 5 questions.

   On Time 1-5
   Good Price 1-5
   Quality Product 1-5
   Likely to tell Others 1-5

   1=no 5=Yes for Sure

   use form to take information
   use reporting tool to have a widget like this

   On Time
   rating of 1 | ---------this is a bar showing results in the aggregate
   rating of 2 | ---------------
   rating of 3 | ---------------------------------
   rating of 4| ------------------------------------------------------
   rating of 5 | ---------------------------------------------------------------------------------

   Percentage of satisfaction with XX Clients Surveyed

   You can use Matrix field for that. And then use the bar chart widget to generate the report.
   When setting up the widget, make sure to select "all rows" as the widget datasource.
