  1. I had to switch hosting companies. I had paid someone to customize my forms. Are they all lost?

  2. You should move all machform files and database from the old hosting to the new one when switch the hosting company.

That would not make you lost the previous forms and data.

  3. We downloaded the machform folder that was on old hosting site and still have it. My husband configured machform for me and couldn't figure out how to get old forms up.

  6. I found a bunch of forms in the data folder within the machform folder I had on previous host server. I saved them and then uploaded to current data folder. Don't see them when I go to admin. Is there something else I need to do?

  7. my husband says he saved the folder but not the database:0
Doe sthis mean that even though I have the forms in the data folder I can't retrieve?

  8. You need to do export the database structure and data from your mySQL database on the old host and import it into the new DB.

Use phpMyAdmin for this, most hosts should supply this utility, else you can download and install it yourself.

All the actual forms, and the form data is stored in the MySQL DB.

I'm sure Yuniar can assist as this would be an installation issue, and AppNitro provides installation support, otherwise I expect your old host can help with the export, and the new host with the import of the data.
