I have a couple forms that when I uploaded the form as a website, there is a strange writing above the form. See below.
How can I get rid of this.

May I know what file editor you use? some editor add special character to your files, you may try using another file editor like Notepad++
Sorry, just now seeing this. It was notepad that I was using.
Hmm.. that's odd. How did you upload it? Are you using FTP or some kind of online WYSIWYG?
