I am a bit lost on the setting up DB thing. I have uploaded the software to my RedHat Linux machine that is running SQL but have no idea (nor access) to create a new DB. Will this software run under a windows 2003 server with web access? If so, maybe I create a new DB there.

MachForm can run under both Linux or Windows server, as long as both having PHP and MySQL database installed.
If you are using Linux, the basic way to create database is through the console.
Easier way is to install PhpMyAdmin and use it to manage your databases.

OK, I have installed PHP, MYSQL and PHPAdmin to no avail. Like I said earlier, this is installed on a Win 2003 server. Anyway, my latest message when trying to run
http://localhost/machform/installer.php is:
The specified procedure could not be found.
Any idea...

Also, in case that doesn't make any sense, could I have you guys try to set it up for me? The server is actually sitting in a private network (no outside IP), if I were to install an outside address, what software would you need to access the box?

I'm afraid it won't be possible to have us setup your server without public IP.
Anyway, are you sure you have your PHP configured properly?
Try to create a simple test PHP script and run it.Or you might want to install WAMPServer instead:
www.wampserver.com/en/This would install all those Apache + PHP + MySQL at once.

OK, I have setup a Ubuntu server that I am going to run from my home network. I will give you an access IP number later today. I would really appreciate your setting this software up if you can. Besides root access, what software would you need opened to allow your access? (VNC, SSH et.)

SSH is enough for us.
However, please note that our installation service doesn't include setting up your Apache + MySQL + PHP, as this is server admin task and beyond our script. Make sure that you have all those installed and running first before submitting MachForm installation request.
