Sending spam from my server!!!

    X-PHP-Script: gicbd.com/request/js/qrcode/config.php

    is sending out spam off my server!!!! How come this is happening?

    That file is not coming from the original machform package. You can delete that file.

    Most likely, your server has been compromised and the attacker put the file there (might be somewhere else as well).
    I suggest to scan your whole website files and reupload machform files from the original package, to ensure the security of your site.
