   I have had no problem embedding the form into a page, but when I generate a report and try the link provided, the page opens but the iframe is blank. Page inspector shows no errors. I can't find any permission issues on the directory that contains the form code.

   What can be wrong?

   The PHP on your server seems to be configured to hide error message, hence you don't see any message and just a blank page.
   To investigate this, you'll need to check into your PHP error log file (usually named "error_log" under your site folder) or adjust your PHP configuration file (php.ini) and set this value:

   display_errors = On

   This way, the error message will be displayed.
   Please let me know the detailed error message.
