If I purchase MachForm do I have to operate from one computer only in order to create or make changes to the form, e.g. I work on my internet site from both my home computer, my laptop when travelling, and from my work computer?

Machform is using web technology, since you install it on your website, you can operate machform anywhere as long as you connected to the internet :)

You install it on your website. As Redityo says, that means as long as you have 'net access, you can change/create the forms
The user licence refers to installation of machform itself on one domain/site, or according to the multiple licence pack you purchase.
