MySQL Version Error - but running MySQL version 5.0.41

    I am getting the following error when I try to install MachForm. However, I have verified that I am running MySQL version 5.0.41.

    MySQL Version >= 4.1.0
    Correct User and Password for MySQL
    Unknown MySQL server host 'DB_HOST' (1)
    Establishing a Connection to Database [DB_NAME]
    Unable to connect to MySQL

    How can I fix this?

    from this error :

    Establishing a Connection to Database [DB_NAME]

    it seems you have put the database information in config.php file on wrong side. You should put the database information on the right side like this :

    define('DB_NAME', 'example_database_machform');
