Im sorry to be so ignorant to this but I just downloaded machform.. What do I do next? I unzipped the folder but do not see where to install it on my computer so I can make the forms I need to be added to my site.
Any help is greatly appreciated!

Read the install file. It tells you what to do. Essentially, you need to upload the files to your web server then start the installation.
e.g. http://www.yourserver.com/machform/install.php
