like when someone clicks on the admissions page my forms pop up they click answer yes to a few questions? and when they hit the submit it will allow access to the page?

Popping up the form itself is quite simple, you can simply set a link to the form using some javascript to create the popup window.
However, limiting the page on your site is quite different. MachForm itself won't be able to restrict/allow your users to visit certain pages on your site.
