How can I insert extra fields to standard fields, for example "name" field?
The standard "name" field got first en last name. I want to insert an extra field for initials.

Do you mean you would like to insert a middle field into the name field?
We have a workaround for this. Please contact us directly via email and we'll send you the files.The next version of MachForm (v3) will support middle name field without any workaround.
