There seems to be a problem with the text under the boxes. Under Internet Explorer there is no text like "First", "Last" etc. It only presents "I" even the submit buttom only have the text "I".
Under Firefox there seems to be no problems. Please take a look at the form:

What version of IE do you use? IE7 or IE8?
If you are using IE8, it's currently Beta version. I suppose there are lot of bugs at this stage.

There is a strange thing going on. It seems that I have this problem on a certain time.

I can't reproduce the same error using multiple computers.
Can you test using another computers?
