What errors are you getting or any other indication that it is not working?

It could be settings in the php.ini file that are limiting upload ability, such as the memory required or execution time or file upload size (different from that set in Machform, but still inter-related since the php.ini setting overrides the Machform file size setting).

Talk to your host about the php.ini settings and if they can be changed/increased, especially specifically the ones above.

Can you give us a test form (try to duplicate the troublesome form and see if it gives the same problem?)

Are any other forms having the same problem?

Yuniar from Appnitro may not see your post on the forum for a few days, so would suggest a direct help request to them - http://www.appnitro.com/contact as that is generally a faster way for them to assist you.

    Thank you for your reply.

This is the test form: http://www.breastfeeding.org.hk/mform/view.php?id=17123

It doesn't upload anything, error message is "Error. This file type is not allowed", while I didn't limit any file type.

I'll look into php.ini file, thanks again.

I would still suggest the error is generated from Machform because inadvertently, you still do seem to have a restriction on file extension types in the File Upload field properties.

The only way I could replicate the error you're getting was by specifying "blocked" file extensions. Try the form below: try a *.txt file, and you will see it won't allow upload.

[edit] a quick look into my own php.ini I couldn't find anything to do with restrictions specifically for file extension types - only file size.

Try to check into your file upload field property. Make sure the dropdown there is being set to "Block" and add these file types:

    php,php3,php4,php5,phtml,exe,pl,cgi,html,htm,js
