I'm getting an error when I'm checking the entries for a particular form. Form #1 is fine, however form #2 is giving me issues. I duplicated the form and get the same error - I created form #3 with just 1 field and that gives no error so not sure what's going on.

Try go to "Entries->Choose columns" menu. Then select some column in there and save it. Maybe the "selected columns" not saved properly.

I have the same issue, I've tried to change the columns but it did not help.
