    I'm receiving the following after submitting:

    There is something odd in your form, do you make any changes to machform files ? I can not see element (input) field in there

  3. I have not made any changes to anything other than what is normal and allowed through the web interface.

  7. I do appreciate your help. You are a new company to me and one not based in the US. Forgive my skepticism, but I am not comfortable sending my business FTP information to you. I'll be glad to send any files you might want to look at.

  8. There is a problem with your database. You need to repair it.

    It looks like the ap_form table might be corrupted (I got this info by following your link).

  9. Yes, Saghalie, I think see what you were referring to. This morning when I went to page with my form, it was missing 90% of the form. It only had the section breaks. I had to login to the admin resave the form, and then all was present on the form again. A filled out a test form and "continued". I still got the same errors I presented at the beginning of this thread.

    As for repairing a database I don't know how to do that. I suppose I could delete everything and start over. I hate to lose my form though.

  10. I must say this is the weirdest thing I ever see.
    I've just checked your form and it lost all input fields indeed.

    Perhaps your mysql user doesn't have full privileges to your database?
    Try to allow all permission for your mysql user to your machform database and then create a new test form.

    I understand if you don't want to share any FTP/Cpanel info.
    However, is there any chance we could login to your MachForm? (not FTP)

  12. I have check my database user and database for permissions. All have been configured for full permissions. I logged in, saved my form, viewed my form and received the same errors.
