I really like how it is on top of the graphic and the fields "wrap" around the image.

Hmm.. I'm afraid this one would be quite difficult to achieve.
It might be possible by manually setting the margin for each of your form fields.
However, I can't promise you would be able to create the exact 100% same layout.
