I just created a second form in my installation at it's ignoring the CSS in the form_# folder. The permissions on the file and folder are exactly the same as those the previous form that works correctly. I've tried copy the form several times to see if that works, but nothing. I even rebooted my desktop and the web server just in case something was hung up somewhere. Any ideas?

Im getting the same issue. When i view source of this new form it reads:
<link rel="stylesheet" type="text/css" href="view.css" media="all" />
If i view source of older forms it reads:
<link rel="stylesheet" type="text/css" href="./data/form_29/css/view.css" media="all" />Also, we just moved to a new server.

Check your config file. You might have missed or changed the data folder path.

Make sure that your new server doesn't run PHP under Safe Mode too.

Its fixed for me. Turnes out the data migration from one hosting to the other did not catch all the files.
