Is it possible to just use the Create/Edit Form screen in our own application ?
We already have a form application but we did not have a GUI tool to add form fields and your tool is just what we need.

The free version of MachForm is available here: http://www.phpform.org. It runs just like MachForm, but only allows you to create/design the form (as you are asking) however, there's no way to save the forms etc, as this is just a demo of the fuller product to get an idea of what it does.
To use this one, you simply go through the wizard presented and then you can copy and paste the code of the created form to your own web page and apply your own form processor script to it.

Tom, you'll definitely need to have your own customization for that purpose.
Which I don't think it would be simple though, since you will need to make it compatible with your current form application.
