Thank you sir but may i know if i follow your instruction to modified it then any file i need to copy together to my company hosting server? Is it all the required file from include? One thing to want to inform you that is your form so nice and so easy to create but when face problem it's will be very diffucult to debug it and also modified....anyway thank your helpfull and i'll try to do it if can't i will ask you one more time.
